Information you give us. When you complete our eligibility survey, we collect your name, phone number, email, Michigan county, who you care for, the care setting, general program and Medicaid status, and optional details such as your current agency or caseworker name. We do not ask for Medicaid numbers, case IDs, or Social Security numbers.

Information collected automatically. We record how you arrived at our site, such as campaign tags (UTM parameters), advertising click identifiers from Meta (Facebook and Instagram), Google, TikTok, and Microsoft, the referring website, the page you landed on, and basic device and browser data.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

We use cookies and similar technologies to remember how you found us and to measure our advertising. Advertising partners such as Meta, Google, and TikTok may set their own cookies. You can block or delete cookies in your browser settings, and you can manage ad preferences directly with each platform.

Sharing with advertising partners. When you submit our eligibility form, we may share hashed (encrypted one-way) contact details, such as your email, phone number, name, and ZIP code, along with technical identifiers such as cookies, IP address, browser type, and ad click IDs, with advertising partners including Meta (Facebook and Instagram) and TikTok. They use this only to measure whether our ads led to a form submission and to improve ad delivery. We do not share your survey answers, health, care, or Medicaid details with these partners.

How to opt out. You can turn off ad personalization in Meta Ad Preferences and in the TikTok app under Settings and privacy → Privacy → Ads personalization. You can also block third-party cookies in your browser, or call us at (248) 876-3530 to ask that your information not be shared with advertising partners.

We share information only with service providers that help us run our business (such as our customer management system, spreadsheet, and messaging tools), with state agencies or care programs when needed to process your enrollment, or when required by law. These providers may use your information only to provide services to us.

We use reasonable administrative and technical safeguards and limit access to trained staff. We keep information only as long as needed for the purposes above or as required by law.

You may ask us to see, correct, or delete your information, or to stop contacting you, by calling (248) 876-3530. Our site is not intended for children under 13, and we do not knowingly collect their information.

We may update this policy from time to time. The date at the top shows when it was last changed.
